Instrument Department Credit Card Authorization Form
If a customer requests a flute(s) to be shipped to an address other than the verified billing address of the credit card, a credit card authorization form will be need to be completed prior to shipping.
The credit card authorization form is necessary to help prevent credit card fraud, as well as for our customers’ safety. Please follow the direction below to download and return the form.

Step 2: There are several different options for returning the form to us:
- Print the form, fill it out, sign it, and then scan the form and email it to us at email@example.com;
- Print the form, fill it out, sign it, and then fax it to us at (248) 855-2525;
- After downloading the form, open it using Adobe Reader, add text and a signature using the Adobe Reader Tools. Then save the file to your computer and email it back to us at firstname.lastname@example.org. (For detailed instructions for this step, see below)
Once this form is received in the store, we can continue processing your instrument order for shipment!
Adobe Reader Instructions: The easiest and most convenient way to complete this form and return it to the Instrument Department is to use Adobe Reader. If you do not already have Adobe Reader installed on your computer, please visit their website for the download.
Follow the instructions from Adobe to install the program. Once the program is installed, there are only a few easy steps that you need to complete!
Step 1: Open the Credit Card Authorization Form that you saved to your computer.
Step 2: Click on the “Sign, add text or send a document for signature” button on the top menu bar:
Step 3: A menu will appear on the right side of the screen called “I Need to Sign.” Click on the “Add Text” option.
You can now click on the lined areas and type text to complete each field. Make sure you don’t forget to add the date at the bottom!
Step 4: Click on “Place Signature” from the “I Need to Sign” menu:
Choose “Draw My Signature” from the options, and then draw your signature in the “Draw Your Signature” box. Then click “accept.”
Step 5: Save the document to your computer by choosing File –> Save from the program’s menu. Then email the document as an attachment to email@example.com
